Job Summary We are seeking a compassionate and dedicated Certified Home Health Aide to join our team and provide exceptional in-home care to clients across diverse settings. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and document patient vital signs accurately while maintaining detailed medical records using Electronic Health Record (EHR) systems or Electronic Medical Records (EMR). Administer medications as prescribed, ensuring proper medication management and adherence to medical terminology standards. Perform specialized care tasks such as catheter care, wound dressing changes, dementia care, Alzheimer's care, hospice support, and end-of-life assistance. Observe patients closely for changes in condition or behavior, including patient observation related to memory care or individuals with developmental disabilities. Maintain patient confidentiality and adhere strictly to HIPAA regulations while managing sensitive medical information. Support clients with meal preparation, light housekeeping, laundry, cleaning, and other home health care assistance to promote a safe and comfortable environment. Communicate effectively with clients and their families about care plans, progress notes, and any concerns related to patient health or safety. Follow home health agency protocols and home & community care standards while ensuring compliance with all relevant regulations. Skills Proven experience working with geriatric patients and individuals with disabilities in home health or long-term care settings. Strong knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (BLS) including CPR certification. Skilled in assisting with ADLs and patient transfer methods using equipment such as Hoyer lifts or other mobility aids. Familiarity with EMR/EHR systems for accurate documentation of patient information and medical records. Excellent patient observation skills to identify changes in condition promptly; experience working with individuals with dementia or Alzheimer's is highly valued. Ability to maintain patient confidentiality per HIPAA guidelines while managing sensitive information discreetly. Effective communication skills for client interaction and collaboration with healthcare teams; experience working within home health agencies preferred. Physical stamina for heavy lifting tasks when necessary; experience providing hospice or end-of-life care is a plus.
