Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support Monitor and observe patients' conditions, noting any changes or concerns related to vital signs, behavior, or health status Administer medications and treatments following medical instructions and home healthcare regulations Support patients with specialized needs including Alzheimer's care, dementia care, end-of-life care, and hospice services Operate Electronic Health Record (EHR) systems to document patient information accurately and securely in compliance with HIPAA standards Use medical terminology appropriately when recording observations or communicating with healthcare professionals Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment for clients Utilize equipment like Hoyer lifts or other assistive devices safely to support patient mobility and prevent injury Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings preferred Demonstrated experience caring for geriatric patients or individuals with disabilities in a home or community setting Knowledge of medical documentation procedures and familiarity with EMR/EHR systems Hands-on experience providing dementia or memory care is highly valued Certification in CPR and Basic Life Support (BLS) required; First Aid certification is a plus Experience assisting with catheter care, medication administration, vital signs monitoring, and hospice/end-of-life care preferred Strong understanding of home health regulations and patient privacy laws such as HIPAA Join us to provide compassionate, energetic caregiving that empowers individuals to live comfortably at home.
