Job Description
Position Summary This position provides patient care to the client in their home under the supervision of a Registered Nurse. Duties include but are not limited to: personal care and homemaking services essential to the patient's health care and comfort at home, including shopping, laundry, meal planning, prep, serving (Rx diet) and assisting with ADL's- bathing, dressing, grooming, toileting, mouth care, hair care, feeding, mobility, transfers, and ambulation. Supports the Mission, Philosophy and objectives of Hunterdon Hospice. Primary Position Responsibilities Follows individualized plan of care as established by the RN. Provides personal care and homemaking services to the patients within the CHHA scope of practice. Documentation of care. Reports unusual circumstances or incidents to the RN. Other duties as assigned.
