Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report any abnormalities promptly Administer medications accurately following prescribed schedules and medical documentation protocols Support patients with specialized care needs such as Alzheimer's disease, dementia, hospice care, or end-of-life care Operate Electronic Health Record (EHR) systems to document patient interactions, medical records, and care plans efficiently and securely in compliance with HIPAA regulations Perform patient transfers using safe methods such as Hoyer lifts or transfer belts while ensuring patient safety and comfort Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, cooking nutritious meals, and ensuring overall hygiene standards are met Provide emotional support and effective communication tailored to each client's needs while respecting their dignity and confidentiality Follow home healthcare regulations diligently to ensure compliance with legal standards and best practices in home & community care Experience Prior experience working as a Home Health Aide or in assisted living facilities is highly preferred Proven background in caring for geriatric patients or individuals with disabilities such as developmental disabilities or memory impairments Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Hands-on experience providing hospice or end-of-life care is a plus Knowledge of patient transfer methods including Hoyer lifts and other mobility aids is desirable Demonstrated ability to work independently while maintaining excellent client communication skills Join us to become part of a dedicated team committed to delivering exceptional home healthcare assistance.
