A Brief Overview Assists patients with personal care and activities of daily living, as instructed by the Registered Nurse and or Physical Therapist. What you will do Assists patient with all aspects of personal care which includes bathing (complete and assisted), hair, nail, skin and mouth care as per assignment sheet Safely assists patients with transfers to bed, chair and/or commode and positions patients appropriately to maintain comfort and prevent skin breakdown Safely assists patients with appropriate rehabilitation exercises as per instructions of Registered Nurse and/or Therapist (PT, etc) Follows a bowel/bladder training program providing incontinence care and assisting with all aspects of toileting and/or catheter care Safely changes occupied or unoccupied bed following policy and procedure Washes patients' personal laundry and bed linen as needed Assists with feeding according to dietary instructions per assignment sheet Assists patients with walking and appropriate use of canes, walkers or wheelchairs instructed Observes the patient's physical and emotional status and informs the Case Manager (Registered Nurse or Physical Therapist) on a timely basis of any changes in the patient's condition or family situation Assists with post-mortem care as instructed by the Registered Nurse Performs accurate recording of intake and output (I&O) and document correctly Demonstrates safe and effective use of Hoyer lift following instructions from Nurse or Physical Therapist Accurately completes documentation and submits completed assignment sheets weekly Incorporates the physiological and developmental age of patients into the overall plan for the delivery of patient care Maintains and respects confidentiality Provides advance notification in for absence and/or tardiness Carries out responsibilities efficiently and effectively in patient care and non-patient care areas Understands and participates in departments' Performance Improvement Plan and process Accepts and completed additional responsibilities as directed by management Education Qualifications High School Diploma Required or GED Required or equivalent experience Experience Qualifications Prior Certified Home Health Aide experience Preferred Knowledge, Skills, and Abilities Certified Home Health Aides must perform duties as trained in their certification program. Strong verbal communication skills Licenses and Certifications CHHA - Certified Home Health Aide NJ Home Health Aide Certification Upon Hire Required
