Job Description
Overview Join our dedicated team as a Home Health Aide or Care Provider and make a meaningful difference in the lives of our elderly and Veterans in need of compassionate, personalized care. In this vital role, you will support patients in their homes or community settings, helping them maintain independence and improve their quality of life. Whether assisting with daily activities, providing emotional support, friendship, or managing activities of daily living, such as meals, hygiene, dressing, bathing, light housekeep your dedication will foster comfort, safety, and dignity for those you serve. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts. Monitor patient health by observing vital signs and reporting changes promptly to healthcare professionals. Support medication schedule and ensure proper documentation. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care and dementia-specific activities. Prepare nutritious meals tailored to dietary restrictions and assist with light housekeeping tasks like cleaning, laundry, and meal preparation to promote a safe living environment. Maintain accurate records and track care by filling out a time card and activity sheet in compliance with healthcare standards and organizational policies. Collaborate effectively with team to ensure comprehensive patient care in home or community settings. Requirements Prior experience in home health care, assisted living facilities, nursing homes, or long-term care environments is preferred. Any certifications such as medication administration, CNA, MA, PCA, in NJ or out of state is a plus. Experience working with individuals with disabilities or developmental challenges is desirable. Excellent communication skills to interact with patients and their families while adhering to HIPAA regulations. Must speak read and write in English Physical stamina to perform caregiving duties that involve lifting, transferring patients, fall prevention, and standing for extended periods.
