In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and well-being. Your expertise will help promote independence and improve quality of life for patients across diverse care settings. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature Administer medications and perform catheter care following medical instructions and home health regulations Maintain accurate medical documentation using Electronic Health Record (EHR) systems and ensure compliance with HIPAA confidentiality standards Support patients with dementia or Alzheimer's disease through specialized memory care techniques and compassionate engagement Assist with transfers using patient transfer methods such as Hoyer lifts or gait belts to ensure safety during mobility tasks Provide end-of-life care with sensitivity, respecting patient dignity and comfort in hospice settings Experience Prior experience working with geriatric patients, individuals with developmental disabilities, or in assisted living environments is highly preferred Demonstrated knowledge of medical terminology, medication administration, and basic life support (BLS) including CPR certification Experience working within home health agencies or providing home & community care services is advantageous Familiarity with EMR/EHR systems for accurate medical records management and documentation Proven ability to communicate effectively with clients and their families while maintaining patient confidentiality under HIPAA guidelines Hands-on experience with patient transfer methods, heavy lifting techniques, and the use of assistive devices such as Hoyer lifts or wheelchairs Knowledge of hospice care principles, dementia/dementia care strategies, and geriatrics best practices Join us in delivering exceptional home healthcare assistance by combining your skills with our mission to provide compassionate, high-quality care.
