Duties Support clients with meal preparation, feeding, and ensuring proper nutrition aligned with care plans Assist clients with Activities of Daily Living (ADLs), including personal hygiene, grooming, dressing, and mobility support Monitor patient health by observing vital signs, administering medications, and reporting changes to healthcare teams Operate patient transfer equipment such as Hoyer lifts and assist with patient transfers using safe manual handling techniques Maintain accurate documentation of care activities, medication administration, and patient observations in compliance with HIPAA regulations Engage clients in meaningful activities that promote mental and emotional well-being, including memory care and dementia support Ensure a clean and safe environment by performing routine cleaning tasks and adhering to infection control protocols Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly preferred Knowledge of patient observation techniques, medication administration, and vital signs monitoring Experience working with individuals with autism, developmental disabilities, neurodevelopmental disorders, or dementia is advantageous Certification in CPR is required; additional certifications such as Hoyer lift training or medication administration are a plus Strong communication skills to effectively interact with clients, families, and healthcare professionals Ability to perform heavy lifting safely and assist with patient transfers using appropriate methods Compassionate attitude with a commitment to respecting residents' rights and promoting independence Join us in providing compassionate care that transforms lives!
