Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report changes promptly to healthcare professionals Administer medications accurately following prescribed schedules and medical instructions while maintaining detailed medical documentation Support patients with specialized care needs such as catheter care, dementia or Alzheimer's care, memory care, and end-of-life support Utilize Electronic Health Record (EHR) systems to document patient interactions, medical records, and care activities in compliance with healthcare regulations such as HIPAA Employ safe patient transfer methods using equipment like Hoyer lifts or patient transfer techniques to prevent injury Maintain a clean and safe environment by assisting with light housekeeping tasks including laundry and meal preparation while respecting infection control protocols Follow home healthcare regulations and safety procedures during all caregiving activities; stay current with certifications such as First Aid and CPR Experience Prior experience working in home healthcare settings or assisted living facilities is highly desirable Demonstrated ability to provide compassionate senior care and support for geriatric patients Experience working with individuals with developmental disabilities or dementia is preferred Familiarity with medical terminology, medication administration, and medical documentation practices is essential Knowledge of patient transfer methods, use of mobility aids like Hoyer lifts, and basic life support techniques such as CPR is required Previous roles involving hospice or end-of-life care are advantageous Strong communication skills for effective client interaction and collaboration with healthcare teams are necessary Join us in delivering exceptional home health care that empowers clients to live safely and comfortably in their own homes.
