Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report any concerns promptly Administer medications following prescribed schedules and medical protocols while maintaining accurate documentation Support patients with medical procedures such as catheter care and wound management under supervision or guidance from healthcare providers Utilize Electronic Health Record (EHR) systems to document patient care activities accurately and securely in compliance with HIPAA regulations Assist with transfers using patient transfer methods like Hoyer lifts or safe manual techniques to ensure safety for both patient and caregiver Provide companionship and emotional support, especially for clients with dementia or experiencing end-of-life care needs; implement memory care strategies when appropriate Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, and meal preparation in accordance with client preferences and safety standards Follow home healthcare regulations diligently while ensuring confidentiality of all patient information at all times Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and update on patient progress Qualifications Proven experience working as a Home Health Aide or in assisted living facilities; hospice or long-term care experience is highly valued Knowledge of medical terminology, medication administration, vital signs monitoring, and basic life support (BLS) including CPR certification Experience working with individuals with developmental disabilities, dementia (including Alzheimer's disease), or geriatrics is preferred Familiarity with EMR/EHR systems for documentation purposes; understanding of home health care regulations is a plus Ability to perform heavy lifting and physical tasks safely; experience using patient transfer equipment like Hoyer lifts is advantageous Strong communication skills to effectively engage with patients, families, and healthcare providers while maintaining patient confidentiality under HIPAA guidelines Compassionate caregiving attitude combined with attention to detail in documenting medical records accurately Valid certification in first aid and CPR; additional certifications related to hospice or dementia care are beneficial This role is an excellent opportunity for motivated caregivers eager to provide exceptional home health services.
