Direct Support Care - Full Time and Part Time and PRN Heart to Heart Home Care is seeking professional, compassionate Direct Support Professionals who are enthusiastic about making a positive difference in the lives of our patients Non Probationary Driver's License High School Diploma or Equivalent One year experience with IDD Direct Support to consumers whether in an In-Home or Group Home setting. Provide support, transportation and assistance for each individual to enable her/him to complete daily routines in a safe, healthy, productive and dignified manner. This usually includes toileting, bathing, dressing, laundry, supervision of recreation. Encourages independence by providing both the opportunity and the appropriate levels of skill training for the individuals in categories such as self-care, household chores, social skills, recreation skills, etc.
