Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report changes promptly Administer medications safely following medical instructions and maintain accurate medication records Support patients with memory care needs, Alzheimer's disease, dementia, and other cognitive impairments through compassionate engagement and structured routines Operate Electronic Health Record (EHR) systems to document care activities accurately and securely in compliance with HIPAA regulations Assist with catheter care, wound management, and other specialized medical procedures as trained Facilitate patient transfers using proper methods such as Hoyer lifts or transfer belts; ensure safety during movement or repositioning Prepare nutritious meals tailored to dietary restrictions; assist with feeding when necessary Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for patients Provide emotional support and effective communication to foster trust and comfort for patients and their families Follow home healthcare regulations and safety protocols diligently to deliver consistent, quality care Qualifications Prior experience working with individuals with disabilities, geriatric patients, or in assisted living or nursing home settings preferred Knowledge of medical terminology, vital signs monitoring, medication administration, and basic life support (CPR/First Aid) certifications required or obtainable upon hire Experience working with clients who have dementia or Alzheimer's disease is highly valued Familiarity with EMR/EHR systems for documentation purposes Ability to perform heavy lifting safely when necessary; experience with patient transfer methods such as Hoyer lifts is advantageous Strong communication skills to effectively interact with patients, families, and healthcare teams Valid driver's license preferred for community-based visits; reliable transportation required where applicable Compliant with home health care assistance standards and regulations including HIPAA confidentiality requirements Demonstrated caregiving skills with a compassionate attitude and energetic work ethic Embark on a rewarding career dedicated to enhancing the quality of life for those in need.
