Responsibilities Assist clients with bathing, grooming, dressing, toileting, and other personal-care needs Support safe mobility, transfers, positioning, and fall prevention Assist with meal preparation and eating as directed by the care plan Provide companionship and meaningful engagement Perform light housekeeping related to the client's care Observe and promptly report changes in a client's condition Follow each client's individualized plan of care Maintain accurate and timely visit documentation Protect each client's dignity, privacy, safety, and independence Communicate professionally with clients, families, supervisors, and the care team Qualifications Active New Jersey Certified Home Health Aide certification in good standing Compassionate and committed to providing excellent care Dependable, professional, and trustworthy Strong communication and interpersonal skills Ability to follow an individualized care plan and agency procedures Respect for clients' individual needs, preferences, cultures, and dignity Ability to complete all required employment screenings, credentialing, and onboarding requirements Reliable transportation is preferred Why Join Concierge Care Collective?
