Job Summary:

We're seeking a motivated and compassionate Driving

CHHA / HHA

to join our team in the Westfield, NJ area. As a certified Home Health Aide, you'll be responsible for providing high-quality care to our clients in their own homes. Responsibilities:

Provide exceptional care to our clients, including assistance with daily living activities, medication management, and personalized care plans Develop strong relationships with clients and their families, ensuring their needs are met and their expectations are exceeded Work collaboratively with our care management team to ensure seamless transitions and coordination of care Maintain accurate records and documentation, adhering to regulatory requirements and company standards Participate in ongoing training and education to stay current with industry best practices and company policies

CHHA / HHA
