Job Summary We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients with diverse needs. The ideal candidate will possess a strong background in patient care, including experience working with geriatric patients, individuals with developmental disabilities, and those requiring specialized dementia or Alzheimer's care. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting, ensuring dignity and comfort. Monitor and observe patients' health status by recording vital signs such as blood pressure, pulse, temperature, and respiration rates. Support medication management by administering prescribed medications accurately following medical instructions and documentation protocols. Provide companionship and emotional support to reduce feelings of isolation and promote mental well-being. Assist with mobility and transfers using patient transfer methods or equipment such as Hoyer lifts to ensure safety for both client and caregiver. Maintain detailed medical records using Electronic Health Record (EHR) systems while adhering to HIPAA regulations to protect patient confidentiality. Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment. Follow home healthcare regulations and protocols related to infection control, safety procedures, and emergency response including first aid and CPR certification. Support clients with specialized needs such as dementia or Alzheimer's care, hospice support, or end-of-life assistance with sensitivity and professionalism. Communicate effectively with clients' families and healthcare teams to coordinate care plans and report changes in health status. Qualifications Proven experience as a Home Health Aide or in a similar caregiving role within home healthcare settings or nursing homes. Knowledge of medical terminology, medication administration, catheter care, and vital signs monitoring. Experience working with individuals with disabilities or developmental challenges is highly desirable. Familiarity with EMR/EHR systems for accurate documentation of patient care activities. Certification in first aid and CPR is required; additional certifications in hospice or dementia care are advantageous. Ability to lift heavy objects safely and perform physical tasks associated with patient transfer methods or equipment use such as Hoyer lifts. Strong interpersonal skills with the ability to communicate effectively across diverse populations while maintaining patient confidentiality under HIPAA guidelines. Demonstrated understanding of home health care regulations, safety standards, and infection control procedures.
