In this role, you will provide personalized assistance to patients across various settings, including private homes, assisted living facilities, and nursing homes. Your adaptability and dedication will ensure each individual receives the highest quality of care, fostering independence and comfort. Responsibilities Deliver comprehensive patient care by assisting with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Observe patients closely for changes in condition and report findings promptly to supervising healthcare professionals Support patient transfer methods using equipment like Hoyer lifts or safe transfer techniques while maintaining patient safety and confidentiality under HIPAA guidelines . Proven experience working with geriatric patients, individuals with developmental disabilities. Excellent communication skills for effective client interaction and collaboration with healthcare teams Experience working with individuals with disabilities or in long-term care environments is highly valued.
