Job Summary We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients across diverse needs. In this vital role, you will assist individuals with daily activities, promote independence, and ensure their comfort and safety. Your enthusiasm and attention to detail will help improve the quality of life for those you serve. Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting Monitor and observe patients' health status, recording vital signs and reporting changes promptly Remind medication administration following prescribed instructions and medical documentation protocols Provide mobility assistance using patient transfer methods and equipment such as Hoyer lifts or gait belts Maintain patient confidentiality in accordance with HIPAA regulations and home healthcare standards Perform light housekeeping duties including cleaning, laundry, and meal preparation to ensure a safe and comfortable environment Document care activities accurately within electronic health record (EHR) or electronic medical record (EMR) systems Skills Proven experience working with individuals with disabilities, dementia, Alzheimer's, or in geriatrics and hospice care Strong knowledge of medical terminology, home health care regulations, and patient safety protocols Ability to perform patient personal care assistance with compassion and professionalism Skilled in taking vital signs, catheter care, medication administration, and basic life support (BLS) including CPR and first aid Familiarity with home health agency operations, home & community care practices, and long-term care environments Excellent communication skills for engaging effectively with clients, families, and healthcare teams MUST speak, read and write English Physical ability to lift heavy objects safely and assist with patient transfers using appropriate techniques Join us in delivering compassionate home health services that empower clients to live safely and comfortably at home.
