Job Summary Join our compassionate team as a Personal Care Assistant (PCA) Home Aid, where your dedication and caregiving skills will make a meaningful difference in the lives of individuals requiring support with daily activities and health management. In this vital role, you will provide personalized home health care assistance, ensuring safety, comfort, and dignity for clients across a variety of settings. Your energetic approach and attention to detail will help promote independence and well-being for those in need of specialized care. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor patient health by observing vital signs, recognizing changes in condition, and accurately documenting medical information in Electronic Health Record (EHR) systems while maintaining strict confidentiality in accordance with HIPAA regulations. Administer medications and provide basic medical care such as catheter care, wound dressing changes, and end-of-life support under the supervision of healthcare professionals. Support clients with memory care needs related to Alzheimer's disease and dementia by implementing specialized techniques for safety and engagement. Prepare nutritious meals aligned with dietary restrictions, assist with light housekeeping tasks including cleaning and laundry to maintain a safe living environment. Communicate effectively with clients, family members, and healthcare teams to ensure coordinated care plans are followed. Follow home healthcare regulations and safety protocols during patient transfers, emergency responses like first aid or CPR, and when working with geriatric or disabled populations. Qualifications Proven experience working in assisted living environments or providing home health care assistance, preferably with geriatric or disabled individuals. Knowledge of medical terminology, medication administration, vital signs monitoring, and basic life support (BLS) including CPR certification. Familiarity with EMR/EHR systems for accurate documentation of patient records and medical documentation. Experience working with individuals with developmental disabilities or dementia care is highly desirable. Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts. Strong communication skills to effectively engage with patients and their families while maintaining patient confidentiality per HIPAA standards. Valid certification in first aid; additional certifications in hospice care or home health agency experience are a plus.
