Job Description
Job Summary We are seeking a compassionate and dedicated Home Health Aide (HHA) to provide exceptional in-home care to clients across diverse settings. As an HHA, you will play a vital role in enhancing the quality of life for individuals requiring assistance with daily activities, medical needs, and emotional support. Duties Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient health status, including vital signs and overall well-being Support medication management and administration following prescribed guidelines Document patient care activities accurately using Electronic Health Record (EHR) systems and maintain confidentiality in accordance with HIPAA regulations Provide specialized care such as Alzheimer's, dementia, memory care, hospice, and end-of-life support Use proper patient transfer methods and equipment like Hoyer lifts to ensure safety during mobility assistance Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for clients Requirements Proven experience working with individuals with developmental disabilities, geriatrics, or in assisted living/nursing home settings Knowledge of medical terminology, catheter care, and basic life support (BLS) including CPR and first aid certification Familiarity with home healthcare regulations, medication administration protocols, and documentation standards Ability to work independently while maintaining excellent communication with clients and healthcare teams Physical ability to perform heavy lifting when necessary and assist with patient transfers using appropriate techniques or equipment such as Hoyer lifts Experience working with EMR/EHR systems for accurate medical documentation Compassionate attitude towards patients requiring memory care or hospice services Valid certification as a Home Health Aide or equivalent; prior experience in home health agency environments is preferred Join us in delivering compassionate home health care that empowers individuals to live safely and comfortably at home.
