Travel between Elizabethtown and Ticonderoga Direct Support Provider - an amazing job working with kids and families! Families First has an amazing cultural of support for our staff, flexibility and growth opportunities. A.General Description/Job Responsibilities:
Direct Support Providers offer directs services provision to children and their families to help children stay at home and in their communities. 1. Direct Support services (including but not limited to the following CFTSS services - PSR, FPSS, YPSS and the following HCBS services - C/FASS, Respite, Pre. Vocational and Supported Employment). Direct Support Services are designed to provide community-based services to children and families who may have difficulty engaging in formal office settings, but can benefit from home and/or community-based services. Direct Support services are goal-directed supports and solution-focused interventions intended to address challenges associated with a behavioral health need and to achieve identified goals or objectives as set forth in the child's treatment plan. Activities provided under Direct Support Services are intended to assist the child/youth and family/caregivers to achieve stability and functional improvement in daily living, personal recovery and/or resilience, family and interpersonal relationships in school and community integration. Will drive personal vehicle to transport participants and/or attend on- or off-site functions and/or meetings B.Education/Training/Experience:
For Direct Support Services Workers:
- Many of these services require a high school diploma with some experience working with youth. A few require an Associate's Degree and some require lived experience. - Salary for these positions will start at $37,200. Relevant experience in serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services. Experience coordinating and participating in team settings. Solid written and verbal communication as well as excellent computer skills. Work effectively and knowledgeably across a broad spectrum of cultural, ethnic, and racial communities. Ability to work in an effective and focused manner when self-directed and beyond immediate oversight. Ability to deliver family-driven, youth-guided services, in the home and community. C.Work Schedule:
Full-time (40 hours per week) while maintaining flexible hours to meet the needs of assigned families as identified by supervisor. These positions often require evening and some weekend work. D.Reporting Responsibilities:
Primary responsibility is to the Director of Family Services, with Clinical focuses being covered by the Manager of Clinical Services and coordination through Health Home Care Managers.Job Type:
