Community Based and HRA Assessment Specialist (Non-Medical)

$25 Per visit. 5 Visits Per day

Monday to Sunday The Community Based and HRA Assessment Specialist is responsible for conducting comprehensive, non-clinical assessments of individuals within their community setting. This role involves gathering social, environmental, and functional information to identify needs, strengths, andresources, and to inform the development of appropriate support or service plans. The Specialist will primarily focus on non-medical factors impacting well-being, such as living situation, access to resources, social support, and daily living skills. Key Responsibilities

Conduct Assessments :

Perform in-person and virtual assessments by utilizing standardized non-clinical tools and structured interviews to collect data on the individual's functional status, social determinants of health, and community needs.

Data Collection and Documentation :

Accurately document all assessment findings, observations, and recommendations in the designated case management system or electronic record within required timelines.

Resource Identification :

Identify and inventory available community resources, programs, and support services relevant to the individual's non-medical needs (e.g., housing, transportation, food security, educational/vocational opportunities).

Collaboration and Referral:

Work collaboratively with case managers, social workers, and other non-medical professionals to facilitate appropriate referrals and ensure continuity of care or support.

Reporting:

Prepare detailed, objective reports summarizing assessment results, highlighting key findings, and proposing practical, non-clinical interventions.

•

Privacy and Ethics :

Maintain strict confidentiality and adhere to all ethical and privacy standards when handling sensitive individual information. Qualifications

•

Education :

High school diploma, Associates degree, Bachelor's degree in Social Work, Psychology, Sociology, Public Health, Human Services, or a related field. Equivalent experience may be considered in lieu of a degree.

Experience:

Previous experience (1-2 years preferred) in coordination, case management, social services, community outreach, interviewing, or data collection in a public-facing role.
