Responsibilities Provide hands-on assistance with activities of daily living, including bathing, grooming, dressing, toileting, and incontinence care Assist with safe transfers and mobility, using proper body mechanics and assistive devices Observe and report changes in patients' physical or emotional condition to the supervising nurse or care team Prepare simple, nutritious meals and assist with feeding when necessary, following dietary guidelines Perform light housekeeping tasks such as laundry, bed making, and tidying to maintain a clean, safe home environment Offer companionship, conversation, and emotional support to reduce isolation and promote well-being Follow individualized care plans and instructions from nurses and other licensed professionals Practice proper infection control, including hand hygiene and use of protective equipment as required Document care provided and patient observations accurately and promptly, using paper or electronic systems Travel between patient homes within the community, managing time and schedules for multiple daily visits Required Skills Personal care assistance (bathing, grooming, dressing) Safe patient transfer and mobility support Vital signs monitoring (temperature, pulse, respiration) Basic infection control and hygiene practices Assistance with medications as directed by nurse or care plan Meal preparation and safe food handling Light housekeeping and home safety awareness Documentation of care and observation reporting Use of home care equipment (walkers, wheelchairs, commodes) Time management and route planning for home visits Basic computer or mobile app documentation skills Effective communication with patients, families, and clinical staff
