VNS Health is seeking a compassionate Home Health Aide to provide personal care and support to patients in their homes across New York City. Responsibilities include assisting with daily living activities, monitoring patient condition, following care plans, and reporting changes to clinical staff. You will promote comfort, safety, and dignity while supporting families and caregivers. Responsibilities Provide assistance with activities of daily living, including bathing, dressing, grooming, toileting, and personal hygiene. Support safe mobility, transfers, and positioning using proper body mechanics and assistive devices as directed. Offer companionship and emotional support to patients, fostering dignity, comfort, and independence. Prepare simple, nutritious meals and assist with feeding as needed according to the care plan. Perform light housekeeping tasks related to patient care, such as tidying patient areas, changing linens, and laundering personal items. Observe and promptly report changes in patients' physical or emotional condition to supervising nurses or appropriate clinical staff. Provide medication reminders only as directed by the care plan and within HHA scope of practice. Follow infection control, safety, and standard precaution protocols in all home environments. Document services provided and patient observations accurately and in a timely manner using agency systems. Communicate and collaborate effectively with nurses, therapists, social workers, and other members of the care team. Travel independently to patients' homes throughout New York City, arriving on time and prepared for scheduled visits. Respect patient confidentiality, cultural values, and individual preferences in all aspects of care.
