This role involves delivering personalized assistance to individuals with diverse needs, including seniors, those with disabilities, or individuals recovering from illness. The ideal candidate will have a strong background in home health care, excellent observational skills, and a genuine desire to improve the quality of life for clients through attentive and respectful caregiving. This position offers six-hour shifts designed to promote consistency and work-life balance while ensuring comprehensive support for our clients' health and well-being. Responsibilities Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support. Monitor and observe patient conditions, including vital signs, behavioral changes, and overall health status. Administer medications accurately following prescribed instructions and document medical records in compliance with HIPAA regulations. Utilize Electronic Health Record (EHR) systems to update patient information efficiently and securely. Support patients with medical procedures such as catheter care, Hoyer lift transfers, and hospice or dementia-related care. Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation. Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for clients. Collaborate with healthcare teams to ensure continuity of care across home & community settings while adhering to medical terminology standards. Skills Proven experience in home health or long-term care environments with familiarity in assisted living or nursing home settings. Strong patient observation skills coupled with the ability to recognize changes in condition promptly. Knowledge of medical documentation practices, HIPAA compliance, and electronic medical record (EMR/EHR) systems. Competence in assisting with ADLs, medication administration, and specialized care such as Alzheimer's or dementia care. Ability to work effectively with individuals with developmental disabilities or disabilities requiring memory care support. Certification in CPR is required; additional certifications such as hospice care or working with individuals with disabilities are advantageous. Excellent communication skills paired with compassionate caregiving approach; capable of working independently within a team environment.
