Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using proper patient transfer methods and equipment such as Hoyer lifts. Monitor and document vital signs accurately, including blood pressure, pulse, respiration, and temperature, adhering to medical terminology standards. Provide specialized dementia care, Alzheimer's care, memory care, hospice support, or end-of-life assistance with sensitivity and professionalism. Support patients with mobility challenges through patient transfer methods and assist with light housekeeping tasks such as cleaning, laundry, cooking nutritious meals, and maintaining a safe environment. Communicate effectively with clients and their families to ensure clarity about care plans and respond promptly to their needs or concerns. Qualifications Proven experience working with individuals with disabilities, developmental disabilities, geriatrics, or in assisted living environments. Knowledge of home health care assistance practices including patient personal care assistance and home & community care experience. Familiarity with medical terminology, medical documentation procedures, and home health regulations. Certification in First Aid and Basic Life Support (BLS), including CPR training. Ability to perform heavy lifting safely when necessary for patient transfers or mobility support. Strong communication skills to effectively interact with patients, families, and healthcare professionals while maintaining patient confidentiality under HIPAA guidelines.
