English Schedule Monday-Sunday 24/7 split-shift coverage available 12-hour day and night shifts available Immediate openings for multiple aides Participant Information Experience caring for individuals with Dementia/Alzheimer's disease is preferred. Must be comfortable using a Hoyer lift for transfers. Provide assistance with personal care, mobility, safety supervision, and activities of daily living while maintaining the participant's comfort and dignity. Requirements Active New York State Home Health Aide (HHA) Certificate (Required) Previous home care experience preferred Experience caring for clients with dementia, Alzheimer's disease, Parkinson's disease, or bed-bound clients preferred Ability to work independently while following an individualized care plan Must be available to work 12-hour day and/or night shifts Must be willing to travel to client homes within assigned service areas based on client needs Reliable transportation is preferred Strong communication and interpersonal skills Job Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, transfers, and mobility assistance Provide supervision to promote each client's health, safety, and overall well-being Follow each client's individualized NHTD Plan of Care Provide compassionate care for clients with dementia, Alzheimer's disease, Parkinson's disease, and those who are bed-bound Assist with repositioning, transfers, and mobility Prepare meals and perform light housekeeping Provide companionship and emotional support Observe, document, and promptly report changes in the client's physical, mental, or emotional condition Immediately report incidents, accidents, emergencies, and safety concerns to the Care365 office Maintain accurate documentation and communicate effectively with supervisors and the interdisciplinary care team Follow all agency policies, HIPAA regulations, and infection control procedures
Qualifications:
Valid HHA certification (required). Compassionate and dependable.
