The Direct Support Professional will assist persons with Developmental Disabilities and Traumatic Brain Injury to develop skills necessary to live independently and successfully in the community of their choice. Habilitation Services include assessment, training, and supervision of an individual with self-care, medication management, task completion, communication skills, interpersonal skills, socialization, sensory/motor skills, mobility, community transportation skills, reduction/elimination of maladaptive behaviors, problem solving skills, money management, pre-vocational skills and skills to maintain a household. Train and supervise individuals we support in the community at volunteer sites, educational activities, or recreational outings. Provide transportation to and from these activities utilizing agency vehicles. Assist with updating and implementing day habi9litation plans for all individuals we support. Complete and ensure accurate and timely completion of daily notes, behavior data sheets, behavioral occurrence forms, incident reports, bus duty, van mileage and daily individuals we support schedules. Assist supervisor to develop new volunteer sites and program activities that are consistent with the needs and interests of program participants. Provide the maximum level of supervision to ensure the health and safety of individuals we support. Transport individuals, as needed. Assist with van duty and lunch coverage. Provide assistance with personal care skills as necessary. Complete annual training (Choking, Incident Report, CPR, Defensive Driving, First Aide, Core Competency, etc). Other duties, as assigned. High School Diploma or Associate Degree in a health or human service field (Bachelor's Degree preferred) 1 year of experience working with people with a developmental disability a valid NYS driver's license and own transportation.
