Job Summary Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of patients receiving home-based medical and personal support. In this vital role, you will assist individuals with daily living activities, monitor health conditions, and provide emotional support to promote independence and well-being. Responsibilities Provide personalized assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and record vital signs such as blood pressure, pulse, respiration, and temperature to track patient health status Assist with medication administration following prescribed instructions and document all medical records accurately using Electronic Health Record (EHR) systems Support patients with specialized care needs such as Alzheimer's care, dementia care, hospice care, and end-of-life care Perform patient observation to identify changes in condition or behavior and communicate findings promptly to healthcare professionals Assist with medical procedures including catheter care, wound care, and basic life support when necessary Support safe patient transfers using equipment like Hoyer lifts or other assistive devices while practicing proper body mechanics and heavy lifting techniques Maintain a clean and safe environment by assisting with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring compliance with home & community care standards Requirements Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred Knowledge of medical terminology, HIPAA regulations, and documentation procedures in EMR/EHR systems Proven ability to work effectively with geriatric patients and individuals with disabilities or developmental challenges Certifications in CPR (Cardiopulmonary Resuscitation), First Aid, Basic Life Support (BLS), and hospice or Alzheimer's/dementia care are advantageous Ability to lift heavy objects safely and perform physically demanding tasks required for patient transfers and mobility assistance Strong communication skills for collaborating with healthcare teams and providing compassionate patient care Valid driver's license and reliable transportation may be required for travel between patient homes Join us in delivering exceptional home healthcare services that prioritize dignity, independence, and comfort.
