Job Description

Champlain Valley Senior Community is seeking compassionate and dependable Home Health Aides (HHAs) to provide person-centered care to residents in our Assisted Living, Enhanced Assisted Living, and Memory Care community.
Responsibilities:
Assist residents with personal care, including bathing, dressing, grooming, toileting, and mobility Provide reminders and assistance according to each resident's individualized service plan Respond promptly to resident needs and call bells Observe residents and report changes in condition to the appropriate supervisor Assist with meals, hydration, transfers, repositioning, and other activities of daily living Complete required documentation accurately and before the end of each shift Maintain residents' privacy, dignity, safety, and independence Follow infection-control procedures and all facility policies Work cooperatively with nurses, medication technicians, resident care assistants, and other team members Participate in required orientation, training, and staff meetings
Qualifications:
Current New York State HHA certification required Compassionate, dependable, and able to work effectively with older adults Experience in assisted living, memory care, or senior care is preferred Ability to communicate clearly and follow written care instructions Ability to perform the physical duties of the position safely Must successfully complete all required background checks, health requirements, and employment documentation
Schedule:
Full-time, part-time, and per-diem opportunities may be available. Availability for evenings, weekends, and holidays may be required based on the position.
