Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs and reporting any changes to healthcare professionals Administer medications and manage medical documentation accurately in electronic health record (EHR) systems Support clients with medical procedures such as catheter care, wound care, and end-of-life care with sensitivity and professionalism Facilitate patient transfers using appropriate methods like Hoyer lifts or transfer belts, ensuring safety at all times Maintain patient confidentiality and adhere to home healthcare regulations and HIPAA standards Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment Requirements Proven experience working with individuals with disabilities, geriatrics, or in assisted living environments Knowledge of medical terminology and familiarity with EMR/EHR systems for documentation Certification in CPR and basic life support (BLS), with first aid training preferred Ability to lift heavy objects safely and perform patient transfer methods effectively Strong communication skills for client interaction and team collaboration Experience working within home health agencies or providing hospice or dementia care is highly desirable Valid driver's license and reliable transportation are preferred for client visits Join us to provide compassionate, energetic care that transforms lives!
