Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature Administer medications and perform catheter care following medical instructions and home healthcare regulations Support patients with memory care needs, Alzheimer's care, dementia care, and end-of-life assistance with patience and empathy Maintain accurate medical documentation using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems; ensure compliance with HIPAA and patient confidentiality standards Assist with patient transfers using proper methods and equipment such as Hoyer lifts or patient transfer techniques Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment Skills Proven experience working with geriatric patients or individuals with disabilities in home healthcare or long-term care settings Strong knowledge of medical terminology, medication administration, and vital signs monitoring Skilled in patient personal care assistance, including hygiene support and mobility aid Familiarity with home health care assistance protocols, hospice care practices, and end-of-life support Ability to operate EMR/EHR systems accurately and efficiently; experience with home health agency procedures preferred Excellent communication skills for client interaction and family updates; compassionate caregiver attitude Knowledge of CPR, First Aid, Basic Life Support (BLS), and heavy lifting safety procedures for patient transfers Join us to provide essential home health services that promote dignity, independence, and quality of life.
