Work Remotely No Overview Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail make a meaningful difference in the lives of individuals needing support at home. In this vital role, you will assist clients with daily activities, monitor their health status, and provide personalized care that promotes independence and well-being. This position offers an engaging environment where your skills in patient observation, medication administration, and home care are valued and utilized to improve quality of life. Responsibilities Provide direct patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary. Monitor and document vital signs, health changes, and patient observations accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure seamless medical documentation. Assist with medication administration following prescribed protocols while maintaining compliance with HIPAA regulations to protect patient privacy. Support clients with specialized needs including dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or disabilities. Perform household tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment conducive to health and comfort. Utilize medical terminology appropriately when recording notes or communicating with healthcare professionals. Operate medical equipment like catheters or Hoyer lifts safely and effectively while adhering to all safety standards. Observe patients closely for any changes in condition or behavior that may require escalation or reporting to supervising nurses or physicians. Maintain detailed records of patient care activities, medication administration, and medical documentation in compliance with HIPAA standards. Collaborate with multidisciplinary teams including nursing staff, social workers, and family members to deliver comprehensive home health services. Experience Prior experience in home health care or assisted living environments is highly preferred. Demonstrated ability to work with seniors and individuals with disabilities in long-term care settings such as nursing homes or hospice environments. Knowledge of medical terminology relevant to patient care scenarios including dementia, Alzheimer's disease, hospice needs, and medication management. Certification in CPR is required; additional certifications such as hospice care training or catheter care are advantageous. Experience working with individuals who have developmental disabilities or require memory care is a plus. Strong understanding of HIPAA regulations ensures confidentiality and privacy in all aspects of patient interaction and record keeping.
