Responsibilities Deliver personal care assistance including bathing, grooming, dressing, and mobility support while maintaining patient dignity and confidentiality. Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and medication reminders to promote patient independence. Monitor and observe patient conditions by recording vital signs like blood pressure, pulse, temperature, and respiration; report any changes promptly. Administer medications following medical documentation protocols and home healthcare regulations under the supervision of healthcare professionals. Use Electronic Health Record (EHR) systems or Electronic Medical Record (EMR) systems to accurately document patient interactions and medical information. Support patients with mobility using transfer methods such as Hoyer lifts or patient transfer techniques; ensure safety during transfers and ambulation. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care strategies and behavioral management. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment aligned with health standards. Assist with end-of-life care or hospice support by offering compassionate companionship and symptom management. Follow all home health care assistance protocols while adhering to HIPAA guidelines to protect patient privacy at all times. Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred. Demonstrated ability to work effectively with geriatric patients and individuals with developmental disabilities or disabilities of any kind. Knowledge of medical terminology, catheter care, wound management, and basic life support (BLS) certification is essential. Experience using EMR/EHR systems for documentation is advantageous; familiarity with home health agency operations is a plus. Proven ability to perform heavy lifting safely using appropriate techniques or equipment such as Hoyer lifts when necessary. Strong communication skills for client interaction and collaboration with healthcare teams while maintaining patient confidentiality under HIPAA standards.
