Responsibilities Assist clients with Activities of Daily Living (ADLs), including personal hygiene, grooming, dressing, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor patients' vital signs and observe for any changes in health status, ensuring timely communication with healthcare teams. Administer medications following care plans and ensure compliance with HIPAA regulations to protect client confidentiality. Prepare nutritious meals and assist with meal consumption, accommodating dietary restrictions and preferences. Support clients during community outings or appointments through personal transportation assistance and ensuring safety at all times. Maintain detailed documentation of care provided, patient observations, medication administration, and adherence to resident rights policies. Engage clients in meaningful activities tailored to their interests and abilities, fostering social interaction and cognitive engagement. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Demonstrated ability to perform patient observation, monitor vital signs, and assist with medication administration accurately. Experience working with individuals with autism, dementia (including Alzheimer's care), intellectual disabilities, or neurodevelopmental disorders is preferred. Knowledge of care plans development and implementation for residents with complex needs such as memory care or hospice care. Familiarity with patient transfer methods like Hoyer lifts and patient personal care assistance techniques is essential. Certification in CPR and First Aid is required; additional certifications related to dementia or hospice care are a plus.
