As an entry level classification, this position does not require previous experience in serving the DD population. Duties Provide supervision, support, and guidance to individuals through their daily routines. Assist client(s) with completion socialization and community participation. Help maintain medical, physical, mental health, and personal care needs, to include but not limited to: Nutrition, Exercise, Social Skills, Communication. Document client cooperation, activities, and behaviors to ensure health and safety of clients is maintained at all times. Monitor the safety of clients. Foster positive behavior at the day program and in the community. Assist with meal preparation and/or feeding. Provide support to clients to learn daily life skills. Monitor and document client activity to ensure information is accurate and consistent and maintains compliance with OISP. Provide personal care, toileting and/or hygiene assistance. Facilitate activities in small group settings on site or in the community. Encourage and foster clients' preferred hobbies and areas of interest. Transport individuals to and from programs, vocational habilitation sites, and recreational activities. Will complete other duties as assigned.
