Job Description

ESSENTIAL FUNCTIONS
Participate in the consumer's Individual Service Plan (ISP) meetings when requested by the consumer's interdisciplinary team. Perform tasks and duties according to the ISP. Shall prioritize and organize tasks to achieve goals outlined on the ISP. Maintain a clean and safe environment and shall be sensitive to the consumer's and family's needs. Recognize changes in the consumer's condition and behavior as well as safety and sanitation hazards, report them to family and case management as appropriate. Provide basic personal care and grooming, including bathing (and bladder/bowel requirements), care of the hair, and assistance with clothing. Perform household services essential to the consumer's health and comfort in the home (e.g. necessary changing of bed linens or rearranging of furniture to facilitate ease of movement in the home). Perform when necessary, homemaking tasks such as cooking, feeding, cleaning, laundry and shopping. Will also be required to accompany consumer to medical appointments, on walks/outings and other like appointments. Must be willing to work flexible schedule to meet the needs of the clients. May be required to stay past scheduled shift end time in the event of emergency. In order to maintain the health and safety of the individuals we serve; may be required to work overtime, whether scheduled or unscheduled. Maintain client confidentiality as required by County DD standards and in compliance with HIPAA regulations.
REQUIREMENTS
High School Diploma or Equivalent One to four years of college in a related field is preferred Valid Ohio Driver's License - with less than 6 points Reliable transportation with proof of insurance CPR/First Aid certification Working Cellular Phone - to clock in and out for billing purposes
SKILLS:
Strong interpersonal skills Effective oral and written communication skills Ability to manage multiple tasks
PHYSICAL REQUIREMENTS
Must be able to lift, carry push or pull a minimum of 50 pounds. Frequently requires sitting, standing, walking, bending, squatting, kneeling and reaching. May be required to be on feet hours at a time.
