Job Summary We are seeking a compassionate and energetic Direct Support Professional to provide essential care and assistance to individuals with diverse needs. Responsibilities Assist clients with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support Monitor patients' health status by taking vital signs and observing for any changes or concerns Implement and follow individualized care plans tailored to each client's needs, including medication administration and health monitoring Support clients during meal preparation, feeding, and nutrition management to promote healthy eating habits Use proper patient transfer methods and manual handling techniques such as Hoyer lifts to ensure safety during mobility assistance Maintain accurate documentation of care provided, patient progress, and any incidents in compliance with HIPAA regulations Provide emotional support and engage clients in social activities to promote mental well-being and community integration Skills Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable Knowledge of dementia care, Alzheimer's care, memory care, and neurodevelopmental disorders such as autism is a plus Proficiency in patient observation, monitoring vital signs, administering medication, and following care plans Ability to perform patient transfer methods safely using equipment like Hoyer lifts and patient handling techniques Strong communication skills for effective client interaction and collaboration with team members Familiarity with HIPAA regulations ensuring confidentiality of client information Basic first aid and CPR certification to respond promptly in emergencies Experience supporting individuals with developmental disabilities or intellectual disabilities support needs Capable of heavy lifting and manual handling required for patient transfers or mobility assistance Knowledge of home & community care practices including meal prep, transportation assistance, and personal care support Join us as a Direct Support Professional to be a vital part of a dedicated team committed to enriching lives through compassionate caregiving.
