Job Description

Renaissance House, Inc. is a private non-profit organization dedicated to providing residential services for individuals with disabilities for over 30 years. The level of assistance is tailored to each person's needs, ranging from daily visits to intensive around-the-clock care. Renaissance House, Inc. has grown to serve many men and women across Erie, Hancock, Huron, Sandusky and Seneca counties in Ohio. In most cases, services are provided to people in their own homes.
Job Description :
Provide services to individuals served as identified in the Individual's Service Plan, accurately documenting services delivered on the required forms. Provide safe transportation to the individuals using either an agency or personal vehicle. Maintain auto insurance coverage and a valid Ohio driver's license. Administer the individuals' prescription and OTC medications according to the agency's Medication Administration policy. Accompany individuals to medical appointments as directed, and provide documentation and outcome of the appointment to the Service Coordinator or Program Specialist. Accurately document the individuals' benefit and personal finances according to the agency's Personal Funds of the Individuals policy. Conduct fire and emergency response drills and safety reviews according to the agency's Fire Control and Emergency Response Plan procedure. Assist the individuals in planning and providing nutritional foods according to the agency's Food and Nutrition Requirement policy. Assist the individuals in meeting their personal care needs when applicable including feeding, dressing, bathing and toileting. Assist individual with maintaining clothing and personal effects according to agency's policy. Assist individuals to maintain a clean, safe and sanitary living environment. Develop and maintain a respectful, caring relationship with the individuals advocating for their needs and requests as appropriate. Assist the individuals in presenting themselves in valued roles to family, friends, and the community. Maintain a professional and friendly demeanor when interacting with individuals served, family, guardians, state and county oversight agencies, and other professional establishments. Provide pet care for clients who own pets. Must be a team player and communicate well with coworkers, supervisors and other management.
Skills/Qualifications:
Valid Ohio Driver's License, acceptable driving record (less than 6 points on record). High School Diploma or equivalent. Ability to relate to and communicate with individuals with disabilities. Ability to read and comprehend written materials Ability to perform basic math functions and ability to manage funds. Ability to legibly write and complete documentation. Skill in organizing, training, and motivating individual served.
Physical Demands:
Requires physical effort or strain. May include lifting, constant stooping and walking, the ability to routinely push/pull, climb stairs, reaching and bending, with the possibility of lifting 50 lbs.
Mental Demands:
Job involves performing tasks under the direction and supervision of the Service Coordinator. Work requires adherence to precise procedures and standards involving a high degree of accuracy in observing, recording and reporting data.
Working Environment:
A home setting with possible exposure to unexpected situations.
Rate of Pay:
$18.00 - $18.50 depending on the individuals home that you will be required to work.
