Responsibilities Assist residents with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor residents' health by taking vital signs, observing patient conditions, and reporting any changes promptly to healthcare teams. Administer medications following care plans and ensure compliance with HIPAA regulations to protect patient confidentiality. Prepare nutritious meals and assist with meal consumption, ensuring dietary needs are met in accordance with individual care plans. Support residents during recreational activities, outings, and community engagement to promote social interaction and mental stimulation. Maintain a clean and safe environment through regular cleaning duties and adherence to health standards in line with home health and hospice care practices. Document care activities accurately in resident records, including patient monitoring notes, medication administration logs, and incident reports. Behavioral Skills Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments. Strong knowledge of dementia care, Alzheimer's care, memory care, and neurodevelopmental disorders such as autism. Skilled in patient observation, vital signs measurement, medication administration, CPR/First Aid certification, and emergency response procedures. Ability to assist with ADLs using appropriate manual handling techniques like Hoyer lifts and patient transfer methods. Excellent communication skills for engaging effectively with residents, families, and healthcare teams. Experience supporting individuals with developmental disabilities or intellectual disabilities with compassionate caregiving approaches. Familiarity with HIPAA regulations to ensure confidentiality and privacy of resident information.
