Job Description
Overview We are seeking compassionate and dedicated Home Health Aides to provide essential support to individuals in their homes. As a Home Health Aide, you will play a vital role in enhancing the quality of life for our clients by assisting them with daily activities and ensuring their comfort and safety. This position requires a strong commitment to patient care and the ability to work effectively with individuals of varying needs, including those with developmental disabilities, dementia, and other health challenges. Responsibilities Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and toileting. Provide companionship and emotional support to clients. Monitor and document vital signs and changes in client conditions. Administer medications as prescribed and ensure proper medication management. Assist with mobility using equipment such as Hoyer lifts when necessary. Maintain cleanliness in the client's living environment, including laundry and meal preparation. Document patient care activities accurately in electronic medical records (EMR) systems. Follow HIPAA guidelines to ensure client confidentiality and privacy. Collaborate with healthcare professionals to develop and implement care plans tailored to individual needs. Provide specialized care for clients with Alzheimer's disease, dementia, or other cognitive impairments. Skills Strong patient observation skills to identify changes in client conditions. Experience in assisted living or nursing home settings is preferred. Familiarity with medical terminology and documentation practices. CPR certification is a plus. Knowledge of catheter care, hospice care, and memory care techniques. Experience working with people with developmental disabilities or disabilities in general is beneficial. Ability to provide compassionate caregiving while maintaining professionalism at all times. Proficiency in cleaning tasks and meal preparation for clients. Understanding of long-term care principles and home & community care experience is advantageous. Ability to work independently while being part of a larger healthcare team.
