Job Overview Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and proactive support will make a meaningful difference in the lives of patients and their families. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, grooming, dressing, and toileting, ensuring respectful and thorough support. Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report any concerns promptly. Support medication management by administering prescribed medications following medical instructions and maintaining accurate documentation. Provide specialized care for patients with Alzheimer's disease, dementia, or other cognitive impairments, including memory care techniques and safety measures. Assist with mobility and transfers using proper patient transfer methods and equipment such as Hoyer lifts or gait belts to ensure safety for both patient and caregiver. Maintain detailed medical records using Electronic Health Record (EHR) systems while adhering to HIPAA regulations to protect patient confidentiality. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment aligned with home healthcare regulations. Support end-of-life or hospice care with compassion, patience, and respect for patient wishes while providing emotional support to families. Communicate effectively with clients and their families to ensure understanding of care plans and foster trustful relationships. Qualifications Proven experience working as a Home Health Aide or in assisted living facilities, nursing homes, or long-term care environments; experience with individuals with disabilities is highly valued. Strong knowledge of medical terminology, patient care procedures, and home health care regulations. Certification in First Aid and Basic Life Support (BLS), including CPR; additional certifications such as hospice or dementia care are a plus. Ability to accurately monitor vital signs and assist with medication administration following prescribed instructions. Familiarity with EMR/EHR systems for documentation purposes; experience working with home health agency systems preferred. Physical ability to perform heavy lifting, assist with patient transfers using Hoyer lifts or other equipment, and stand or walk for extended periods. Excellent communication skills for client interaction and team collaboration while maintaining patient confidentiality per HIPAA standards.
