As a Home Health Aide/ Direct Support Professinal, you will play a crucial role in enhancing the quality of life for our clients by delivering personalized care in their homes and community. This position requires a commitment to patient care, observation, and maintaining a safe and clean environment. Duties Assist clients with daily living activities such as bathing, dressing, grooming, and meal preparation. Provide patient observation and report any changes in the client's condition to the supervisor or guardian. Monitor and record vital signs as needed, ensuring accurate documentation in service documentation while adhering to HIPAA regulations. Assist clients with mobility and transfers as needed, including lifting when necessary, to promote independence and safety. Engage clients in memory care activities to stimulate cognitive function and enhance emotional well-being. Maintain a clean and safe environment for clients by performing light housekeeping duties as required. Provide companionship and emotional support to clients, fostering a trusting relationship. Transporating clients to community activities/or to their day program Requirements Previous experience in patient care or as a Home Health Aide is preferred but not mandatory. Certified in CPR techniques is highly desirable. Strong observational skills to recognize changes in patients' conditions effectively. Ability to lift and assist clients safely while promoting their dignity and independence. Understanding of HIPAA regulations regarding patient confidentiality and medical records management. Excellent communication skills, both verbal and written, to interact effectively with clients, families, and healthcare professionals. Compassionate nature with a genuine desire to help others improve their quality of life. Must pass a drug screening test. Must pass background check.
