Overview Join our compassionate team as a Home Health Aide & Caregiver, where your dedication and skills will make a meaningful difference in the lives of individuals requiring personalized care. In this vital role, you will provide essential support to clients in their homes or community settings, promoting independence, comfort, and well-being. Your energetic approach and attention to detail will help foster a safe, respectful, and nurturing environment for those in need of assistance with daily activities and health management. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient condition through observation of vital signs and overall health status Administer medications accurately following medical instructions and maintain detailed medical documentation Support clients with medical procedures such as catheter care, wound care, and managing medical equipment like Hoyer lifts Maintain patient confidentiality by adhering to HIPAA regulations and safeguarding sensitive information Document patient progress using Electronic Health Record (EHR) systems and ensure compliance with home healthcare regulations Provide companionship and emotional support, especially for clients with dementia or experiencing end-of-life care needs Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment Support patient transfer methods safely using proper techniques and equipment such as patient lifts or transfer belts Respond promptly to emergencies by applying first aid or basic life support (CPR) when necessary Skills Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring hospice or memory care Strong knowledge of medical terminology, medication administration, and vital signs monitoring Skilled in patient observation, communication, and providing personalized care tailored to each client's needs Familiarity with EMR/EHR systems for accurate documentation of patient records and medical history Ability to perform heavy lifting safely and operate assistive devices such as Hoyer lifts or mobility aids Excellent understanding of home health care regulations and compliance standards including HIPAA guidelines Experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly desirable Capable of maintaining a compassionate attitude while managing multiple responsibilities in fast-paced environments Knowledge of dementia care techniques, hospice practices, and end-of-life support strategies Join us to be part of a dedicated team committed to delivering exceptional home health care assistance. $15.00 - $17.00 per hour
