Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and observe patient conditions regularly, documenting changes accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations and patient confidentiality Perform vital signs assessments such as blood pressure, pulse, temperature, and respiration rates to track health status Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments; assist with memory care routines Help with catheter care, wound care, and other medical procedures as trained and authorized Facilitate safe patient transfers using proper methods and equipment like Hoyer lifts or transfer belts; assist with mobility and positioning Support end-of-life or hospice care with compassion, dignity, and respect for patient wishes Prepare nutritious meals aligned with dietary restrictions; assist with light housekeeping tasks including cleaning and laundry to maintain a safe environment Communicate effectively with clients and their families to ensure understanding of care plans and address concerns promptly Qualifications Proven experience working as a Home Health Aide or in assisted living, nursing homes, or long-term care settings Knowledge of medical terminology, medication administration protocols, and home health care regulations Demonstrated ability to perform patient observation accurately and recognize changes in condition Familiarity with EMR/EHR systems for documentation purposes Certification in First Aid and Basic Life Support (BLS), including CPR training; additional certifications such as hospice or dementia care are a plus Ability to lift heavy objects safely and perform physical tasks required for patient transfer and mobility assistance Experience working with individuals with developmental disabilities or geriatrics is highly desirable Strong communication skills to effectively interact with patients, families, and healthcare team members Commitment to maintaining patient confidentiality in accordance with HIPAA standards Join us in providing compassionate home healthcare that empowers individuals to live healthier, happier lives.
