Job Description
Join Sevita as a Direct Support Professional in our Healthcare & Medical Services team. In this rewarding role, you'll provide person-centered support to individuals with diverse needs in home and community settings. Assist with daily living, medication reminders, transportation, and community engagement while promoting independence, dignity, and safety. You'll implement care plans, document services, and collaborate with families and clinical teams. Responsibilities Provide direct support with daily living activities (hygiene, meals, mobility). Follow and implement individualized care and behavior support plans. Ensure safety, dignity, and respect in all interactions. Support community outings, appointments, and social activities. Offer medication reminders per policy and training. Document services, progress notes, and incidents accurately and timely. Communicate effectively with supervisors, families, and clinical team members. Monitor individuals for changes in condition and report promptly. Promote independence, skill-building, and community integration. Participate in ongoing training and professional development.
