Responsibilities Observe and document patient conditions meticulously, including vital signs such as blood pressure, pulse, respiration, and temperature. Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts. Support medication administration under supervision, ensuring proper documentation in Electronic Health Record (EHR) systems while adhering to HIPAA regulations. Provide specialized care for residents with dementia or Alzheimer's disease by implementing memory care strategies and maintaining a safe environment. Help with medical procedures such as catheter care and hospice support to ensure comfort and dignity. Conduct patient observations to identify changes in health status or behavior, communicating findings promptly to nursing staff. Assist with meal preparation and feeding to promote nutritional well-being and coordinate with dietary staff as needed. Perform light housekeeping tasks including cleaning patient areas and laundry to maintain hygiene standards. Support patients during transfers and mobility exercises using assistive devices like Hoyer lifts or wheelchairs. Maintain accurate medical documentation and records of patient care activities within EMR (Electronic Medical Records) systems. Engage compassionately with individuals from diverse backgrounds, including those with developmental disabilities or requiring long-term care. Participate in training sessions on CPR certification, infection control protocols, and other safety procedures to ensure optimal patient safety. Requirements Proven experience working as a Nursing Assistant or in similar caregiving roles within nursing homes, assisted living facilities, home health agencies, or community care settings. Strong knowledge of medical terminology relevant to patient care and documentation practices. Demonstrated ability to perform patient observation accurately and respond effectively to changing conditions. Experience assisting with ADLs and providing dementia or Alzheimer's care is highly desirable. Familiarity with EMR/EHR systems for documentation purposes; ability to navigate medical records efficiently. Valid CPR certification; additional certifications such as medication administration or hospice care are advantageous. Ability to work collaboratively with healthcare teams while maintaining professionalism under HIPAA guidelines. Physical stamina for standing, walking, lifting patients or equipment throughout shifts; flexibility to work various shifts including live-in arrangements if required.
