Overview We are seeking compassionate and energetic Home Health Aides to join our team in providing exceptional in-home care. This vital role involves supporting patients with daily activities, ensuring their comfort, safety, and dignity while fostering independence. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to promote independence and comfort Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation using Electronic Health Record (EHR) systems Support medication administration under supervision, including reminding patients to take medications and recording doses accurately Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral support Help with mobility and transfers using patient transfer methods and equipment like Hoyer lifts or other assistive devices Perform light housekeeping tasks such as cleaning, laundry, and meal preparation to maintain a safe and healthy environment Follow home healthcare regulations, maintain patient confidentiality according to HIPAA standards, and ensure compliance during all caregiving activities Skills Strong patient observation skills to detect changes in health status promptly Prior experience in assisted living facilities, nursing homes, or home healthcare agencies preferred Knowledge of medical terminology, catheter care, wound care, and basic life support (BLS) including CPR and first aid certification Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Experience working with individuals with developmental disabilities or geriatrics; dementia or hospice care experience is a plus Ability to perform heavy lifting safely when necessary and use proper patient transfer techniques Excellent communication skills for effective client interaction and teamwork within multidisciplinary care settings Join us in providing compassionate home health care that truly makes a difference!
