Job Description

Job Overview We are seeking compassionate and dedicated Certified Home Health Aides to provide exceptional in-home care to clients across various settings. This role involves assisting individuals with daily activities, ensuring their safety, and promoting independence. The ideal candidate will have a strong background in patient care, experience with senior and disabled populations, and a commitment to improving quality of life through personalized support. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting Provide medication reminders Monitor and observe patient conditions, including vital signs and overall well-being Support clients with mobility using equipment such as Hoyer lifts or transfer belts Prepare nutritious meals and assist with meal planning according to dietary needs Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment Document care activities accurately using Electronic Health Record (EHR) or EMR systems in compliance with HIPAA regulations Support emotional well-being through companionship and engaging activities tailored to individual needs Follow care plans developed by healthcare professionals and communicate any changes or concerns Requirements Home Health Aide Certification Prior experience in home health, assisted living, nursing homes, or long-term care preferred Knowledge of medical terminology and medical documentation practices Certification in CPR or First Aid required or willingness to obtain upon hire Experience working with people with developmental disabilities, dementia, Alzheimer's, or memory care is highly desirable Ability to work independently while following detailed care plans and protocols Strong interpersonal skills with patience, empathy, and excellent communication abilities Ability to work flexible hours including live-in shifts if necessary Familiarity with EMR/EHR systems and HIPAA compliance standards Physical ability to assist with patient mobility tasks such as transfers or lifts Valid driver's license may be required for certain positions involving community-based care This role is vital in supporting individuals to maintain their independence at home while ensuring safety and comfort. $16.00 - $18.00 per hour
