Job Summary Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of our clients. In this vital role, you will provide personalized assistance to individuals in their homes, supporting their health, safety, and well-being. Your energetic approach and commitment to excellence will help promote independence and comfort for those you serve, ensuring they receive top-quality care in a respectful and nurturing environment. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and document patient health status through patient observation and vital signs recording using Electronic Health Record (EHR) systems Administer medications and perform catheter care following medical instructions and home healthcare regulations Support patients with memory care needs such as Alzheimer's disease and dementia, ensuring safety and comfort during daily routines Help with meal preparation, light housekeeping, laundry, and maintaining a clean, safe living environment Transfer patients safely using patient transfer methods like Hoyer lifts or other assistive devices while adhering to proper techniques Communicate effectively with clients, families, and healthcare teams to ensure seamless care delivery and maintain patient confidentiality in compliance with HIPAA standards Skills Strong patient observation skills with the ability to recognize changes in condition or behavior Experience working in assisted living facilities, nursing homes, or home healthcare environments Proficiency with EMR/EHR systems for accurate medical documentation and record keeping Knowledge of medical terminology, medication administration protocols, and basic life support (BLS) procedures including CPR and first aid Familiarity with home health care assistance for geriatric patients, individuals with disabilities or developmental disabilities, and hospice/end-of-life care needs Ability to perform patient transfers using various methods safely; experience with Hoyer lifts preferred Excellent communication skills for engaging with patients compassionately while maintaining professionalism and confidentiality Join us in providing exceptional home health care that empowers individuals to live comfortably and independently.
