Job Description
Job Summary Join our dedicated team as a Home Health Aide & Direct Support Professional, where your compassionate care and proactive support make a meaningful difference in the lives of individuals requiring assistance with daily activities and medical needs. In this vital role, you will provide personalized home healthcare services, ensuring safety, comfort, and dignity for clients across various settings. Your energetic approach and attention to detail will foster independence and enhance quality of life for those you serve. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health through observation of vital signs and overall condition, promptly reporting changes to healthcare providers Administer medications accurately following prescribed instructions while maintaining detailed medical documentation Support clients with specialized care such as Alzheimer's, dementia, memory care, hospice, and end-of-life needs with compassion and patience Operate Electronic Health Record (EHR) systems to document patient interactions, medical records, and care plans securely in compliance with HIPAA regulations Use patient transfer methods safely, including Hoyer lifts and other assistive devices, ensuring proper techniques to prevent injury Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment in accordance with home healthcare regulations Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred Demonstrated knowledge of medical terminology, catheter care, and basic life support (BLS) or CPR certification required Experience working with individuals with developmental disabilities or disabilities-related needs is advantageous Familiarity with EMR/EHR systems for accurate documentation and client communication is desirable Proven ability to provide patient personal care assistance with empathy and professionalism in diverse settings Strong understanding of geriatric care principles, hospice care protocols, and dementia or Alzheimer's care best practices Ability to lift heavy objects safely and perform patient transfers using appropriate techniques such as Hoyer lifts Join us in delivering exceptional home health care that empowers clients to live comfortably and independently.
